NATIONAL FRAMEWORK FOR NHS CONTINUING HEALTHCARE AND NHS-FUNDED NURSING CARE - NOV 2012 (REVISED)

The framework sets out the principles and processes for Continuing Healthcare and NHS-funded nursing care. Every sentence should be read carefully to ensure accurate understanding, including the Practitioner Guidance notes.

NATIONAL FRAMEWORK FOR NHS CONTINUING HEALTHCARE AND NHS-FUNDED NURSING CARE - 2018

Clinical Commissioning Groups must implement the new National Framework between 1 April 2018 and 1 October 2018.  Check with the relevant CCG which framework they are working with before you go through the assessment/review process.

NHS CONTINUING HEALTHCARE CHECKLIST

The Checklist is used for new applicants for NHS Continuing Healthcare and NHS-funded Nursing Care. Advisory note - the Checklist is not an indicator of the likelihood for eligibility. It is a screening tool to identify people who need a full assessment.

NHS CONTINUING HEALTHCARE FAST TRACK PATHWAY TOOL

The Fast Track tool is used for people with a rapidly deteriorating condition and the condition may be entering a terminal phase.  For the purposes of Fast Track eligibility this constitutes a primary health need.  No other test is required.  

DECISION SUPPORT TOOL FOR NHS CONTINUING HEALTHCARE - JUNE 2016

The national tool for gathering evidence to support decision making for Continuing Healthcare.  It contains important user notes and descriptors for each of the assessed domains. Alterations are possible with the new National Framework so clarify with the appropriate CCG.

WEST HAMPSHIRE CCG: NHS CONTINUING HEALTHCARE JOINT OPERATIONAL POLICY

This is the agreed policy for West Hampshire CCG and the four Hampshire CCG's that they work on behalf of. Assessments and reviews must be done in accordance with this policy and the National Framework within these CCGs. We anticipate it will be updated when they implement the new framework.

NHS ENGLAND: OPERATING MODEL FOR NHS CONTINUING HEALTHCARE

This Operating Model sets out the strategic importance of NHS Continuing Healthcare. Annex 2 provides the Improvement Framework focused around the "6Cs" - Care, Compassion, Competent, Communication, Courage and Commitment. (Annotated document)

This document is intended to ensure "that staff have the basic operational information they need to carry out their responsibilities in relation to NHS Continuing Healthcare. It is not a substitute for reading the National Framework". (Annotated document)
