Throughout the review process for Continuing Healthcare that was started in October 2017, there have been over one hundred communications between ourselves and West Hampshire CCG up to March 2018. This has been necessary due to the challenges that we have faced.

In order to help others to be prepared for challenges that may be faced, copies of the complaints and responses received can be found below. As we recorded the meetings, we are confident that the information we have provided is accurate.

At this time, the complaint is on-going and we have yet to receive a response to our most recent correspondence. It will be posted here once we have received it. 

The responses that we received so far have to be read very carefully. In our view, they are inaccurate, have omissions and are deliberately misleading in order to conceal the true motives of the CCG. Despite this, due to our morals, we have removed the names of individuals from the documents below.

ERRORS WITHIN REVIEW REPORT - NOV 2017

The review report from November 2017 contained many serious errors and omissions. This document highlights these inaccuracies. With have a recording of the meeting, available on request, to confirm the information within this document.

EVIDENCE THAT THE REVIEW WAS CALLED CONTRARY TO POLICY

These emails demonstrate that the review was called in contravention of West Hampshire CCG's policies. They also demonstrate that the information given to us was misleading, therefore invalidating our consent.

EMAIL SENT BY US ON 10 DECEMBER 2017 EXPRESSING CONCERNS ABOUT NOVEMBER REVIEW REPORT

We expressed our concerns on 10 December 2017. The CCG failed to act upon our concerns, in direct contravention of the NHS Constitution.

EVIDENCE THAT THE OUTCOME OF FEBRUARY 2018 COMPLAINT WAS PRE-DETERMINED

This demonstrates that the CCG decided the outcome of a complaint regarding process was made prior to the investigation based upon a clinical decision. No consent had been given for the clinical review by Pauline Dorn. The CCG refused to answer questions about this.

SUBJECT ACCESS REQUEST - EMAILS FROM FRIMLEY PARK HOSPITAL

We were informed by West Hampshire CCG that Frimley Park Hospital refused consent to release their documents for a Subject Access Request. These emails refute the allegation. West Hampshire CCG has refused to answer questions about this.

MISSING DOCUMENTS FROM SUBJECT ACCESS REQUEST

This list identifies documents that we know are missing from our subject access request. West Hampshire CCG have had an opportunity to respond but have declined to do so.
